Dental/medical insurance:

We are in-network with Delta Dental PPO however we are not a provider for any other insurance company and therefore considered an “out-of-network” provider.  We will however submit insurance claims for reimbursement of any covered service for you and request that your insurance company send their reimbursement checks directly to the insured policy holder.

A pre-determination of benefits may be obtained prior to treatment for better knowledge of reimbursements and this may take up to 4-6 weeks depending on the specific insurance company.

Our office does not contract with any Health Maintenance Organization (HMO) or Dental Health Maintenance Organization (DHMO) dental plans.

We are not a Medicare provider therefore any of our services can not be billed to Medicare by you or by our office.

Please note: Payment is due at the time of service.

As a courtesy to you, we will bill your insurance company and track claims and instruct them to remit payment directly to the insured policy holder.  Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks.
